2011 California Healthy Kids Survey
Every two years, students in the 5th, 8th, and 11th grades participate in a state-wide survey to gauge the health and wellness of our youth. This September, 5th grade students in our district will participate. Participation requires parent permission.
Our district has three parts to the survey. Part 1 is the California Base Survey, taken by all 5th grade students in California. Part 2 is a survey created for all 5th grade students in Sonoma County. The last part of the survey provides information specific to Measure O, a measure passed by Santa Rosa residents that provides gang prevention services to youth in our city. Below are copies of the survey for your review.
